Individual Counselling Contract and Policy Agreement
After your initial assessment and before ongoing counselling.

Working with Rosa
Before beginning ongoing counselling or supervision, you will be asked to read and agree to the relevant contract and practice policies. This helps ensure that we understand the arrangements, boundaries and responsibilities involved in working together.
The agreement is separate from your confidential assessment. Ask questions before proceeding; Rosa will provide the appropriate agreement at the right stage.
Use your mobile phone camera to scan the QR code. This will open the relevant Microsoft Form, where you can read and complete your agreement. Please complete the agreement that relates to the service you are receiving. If you are unable to scan the QR code, please contact Rosa for assistance.
For couples counselling, each partner must scan the couples counselling QR code and submit their own separate agreement.
After your initial assessment and before ongoing counselling.

Each partner scans this code and submits their own separate agreement before couples work proceeds. Sessions are 60 minutes, £80 per appointment.

Before the supervision relationship begins.

Before student supervision begins, with your course requirements discussed.

Read your service’s contract and policies, including confidentiality and its limits, boundaries, cancellations, annual leave, safeguarding, complaints, referrals and practical contact arrangements. Each partner in couples counselling completes their own agreement; one partner cannot agree for the other.
An acknowledgement checkbox or typed name is not a legally verified digital signature. Please keep any confirmation Microsoft Forms provides after you submit.
No contract acceptance or confidential assessment details are collected on this website. Existing signed agreements are not replaced automatically by changes to website summaries.
